Conference attendees are requested to complete the Participant Registration Form (xls) and send it to isse@pwr.edu.pl as soon as possible. Based on this form you will receive an invoice for the conference fee where you will be asked to pay by bank transfer (payment should be done before 17th of April, 2019). You will be notified by email about the successful transfer.

The file name of the participant registration form should be formatted in the following way: ID_isse2019_registration_form.xlsx.
Where ID is your participant ID received in e-mail.
Example: P001_isse2019_registration_form.xlsx
